Referral Nurse Specialist
Job Type: Full Time
Noble Visiting Nurse and Hospice Services is a full service home healthcare and hospice agency serving Hampden County. We are dedicated to providing high quality, compassionate care to patients in the comfort of their own home.
The responsibilities of this role:
Initiate, build and maintain strong relationships within the community based on trust, knowledge and service. Meet or exceed referral targets. Identify and attend community outreach events and opportunities. Network within the medical and senior community to support ability to achieve results. Maintain focused follow-up and consistency in meeting with referral sources. Provide the highest level of customer service throughout the entire referral and patient service process to ensure continuity of care. Participate in seminars, events and health fairs as needed.
Job Requirements:
Minimum of 2 years sales experience in home care and hospice. Proven abilities to achieve and grow business by calling on physicians, discharge planners, case managers, senior facilities or the like. Extensive and proven experience in relationship building and sales. Current and existing relationship building and sales. Current and existing relationships with referral sources such as physicians, discharge planners, case managers, senior facilities, etc. Knowledge and understanding of home care and hospice regulations. Strong presentation, communication and computer skills, both written and verbal. Strong analytical, problem resolution, decision making skills. Self-motivated and self-starting. Ability to handle multiple priorities simultaneously. Dynamic, energetic, upbeat and approachable. Registered Nurse licensed in Massachusetts with a BSN.
Physical Therapist
Full-time opportunity is available for a Physical Therapist looking to use their skills in a caring, independent and flexible, supportive environment. The therapist will be responsible for providing therapy services and planning/administering medically prescribed therapy treatment programs for patients; consults with medical and professional staff to plan and coordinate joint patient management objectives. Requires a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization. Must be licensed to practice within Massachusetts.
