The home health mission of Noble Visiting Nurse and Hospice Services, Inc., is to provide comprehensive quality home health care and support services, for the purpose of restoring, advancing, and maximizing the level of independence to individuals and families in their place of residence. We believe that all patients will thrive in the comfort of their own home and attain the highest quality of life while respecting their need for dignity and compassionate care.

The hospice mission of the Noble Visiting Nurse Services, Inc., is to provide continuity in care for the terminally ill and their families. We strive to maintain the comfort and dignity of the patient through palliative care with relief of physical symptoms and provision of emotional and spiritual support. We value the quality of life to the end of life, while recognizing the ongoing needs of the patient, family, and staff with emotional and bereavement support.

Compassionate Care Comes To You

If you or a loved one suffers from a terminal illness, finding advanced care without leaving home may seem difficult.  Through Noble Health System's Visiting Nurse and Hospice Services, patients receive individualized care in the comfort of their own home.

Visiting Nurse and Hospice Services offers hospice care with relief of physical symptoms and provision of emotional and spiritual support.  An interdisciplinary team works together to determine a care plan appropriate for each patient.

"Our experienced licensed nurses are trained in managing pain and symptoms that accompany any illness," says Jennifer LaPierre, RN, CHPN, BC, Clinical Manager of Noble Visiting Nurse and Hospice Services.  "We address the individual needs of both the patient and family to ensure the most comfortable care possible."

Easing the Transition

Hospice services are available for patients who have life-limiting illnesses, such as cancer, stroke, heart disease, kidney disease, multiple sclerosis, amyotrophic lateral sclerosis (ALS), and Alzheimer's disease.

"Many people have the misconception that hospice services are only for patients with cancer," says David Mol, MBA, Executive Director of Noble Visiting Nurse and Hospice Services.  "Hospice services provide palliative care for all patients whose prognosis is terminal, with life expectancy of six months or less.  Noble Visiting Nurse and Hospice is here to help you care for yourself or someone with a life limiting illness.  We provide supportive services that help provide comfort care to patients and offer a wide variety of supports to family members.  Hospice is not about giving up on life but maximizing the quality of life, especially for those moments yet to be shared."

Services available through hospice include:

  • nursing by certified hospice and palliative care licensed registered nurses
  • board certified pain management nurse on staff
  • home visiting physician
  • nutritional counseling
  • occupational and physical therapy
  • social services
  • volunteer
  • home health aide
  • massage and music therapy
  • bereavement counseling
  • home delivery of medication and medical equipment

Referrals to hospice or visiting nurse services may come directly from the patient, family, or physician.  Upon referral, an informational visit can be made to answer any questions that the patient or family might have.  Working as a team, our staff partners with a patient's primary care physician to establish an appropriate plan of care.

A Familiar Face

Care can also be provided to patients that are being discharged from hospitals, rehabilitation or long term care facilities through our home care program.

"These services will help avoid re-hospitalization or nursing home placement.  In most cases the patient must be 'homebound'," states Mol.  "This does not mean the patient is bed bound or in a wheelchair but does mean that leaving the home requires a great effort and that the patient rarely leaves home except for appointments such as physician visits.  These patients do not have to have a terminal prognosis to receive care."

Visiting nurse care is provided to people who have a medical condition that can be helped by skilled nursing services.

"Services include wound care by our certified wound nurse, assist with medications, strengthening exercises and conditioning by our rehab team," LaPierre says.  "Our main goal, through both our visiting nurse and hospice services is to maintain a patient's comfort and dignity while continuing to meet his or her physical, emotional and spiritual needs."
